If I have a patient discharged from the hospital and my clinic does all the requirements and charges a TCM, how do I handle that TCM charge if my patient is admitted back the hospital? Example. Patient is admitted to hospital on 6-19 and discharged 6-20. Then the provider contacts patient and they come to clinic on 6-24 and provider charges a 99496. Now patient is readmitted to hospital (for same reason) on 6-27. This is a match for my 72 hr rule. If this had been an office visit on 6-24, I would have attached a PD modifier. How do I handle it since it is a TCM code? I found online that the PD modifier cannot be used with the TCM codes. Do we have to change that TCM back to an E&M? Or do the 72 hr rules not apply to TCM's? I have found all sorts of information online about TCM's and 72 hr rule, but none for this particular situation. HELP!!
