For the transitional care management services, does both criteria of communication with patient within 2 business days of discharge and face to face visit within 7 calendar days of discharge both have to be done or can it be just one? I take it as both need to be done.

Per CPT, the requirements to bill CPT code 99496 for transitional care include communication with the patient within 2 business days of discharge, MD of high complexity, and a face-to-face visit within 7 calendar days of discharge. All of these requirements must be met and documented to bill this service.
