reading CMS guidelines for coding/billling TCM. I just want to make sure i am reading this correctly. I noticed for some of our providers, we are unable to locate the outreach being done within 2 business days.
so, if we are unable to locate the outreach being done within 2 business days, no discharge date in the medical record, only face to face visit and MDM, are we okay to bill TCM since CMS says " at a minimum" HELP??
CMS guidelines:
At a minimum, document this information in the patient’s medical record:
● Patient discharge date
● Patient or caregiver first interactive contact date
● Face-to-face visit date
● Medical decision making (moderate or high)
