We are just diving into the use of the TCM codes 99495 & 99496. There has been some discussion on the facility charges for these. Do facility charges always have to match? Could the facility bill something from the 99212-99215 series? This is a hosp based oncology practice independent of the health system that runs the hosp & their staff does the facility billing.
