I have the scenario of a patient that is in the hospital setting greater than 8 hrs. on the same day. The resident performs H&P note and a separate discharge note. The attending/teaching physician documents an attestation on the H&P - “I saw and examined the patient with the resident and agree with the resident note. (and includes physical and MDM documentation)”. However he only co-signs the discharge note.
