Hello,
Are there any changes in billing for TEE's in 2013? We have started to get denials from Health America with a remark code stating" service not payable w/o additional supporting documentation." We have been billing the TEE's with the CABG's using modifier's -26, -59 and it has not been an issue. We are using CPT 93312. Thank you.
 
What about putting as additional note on the claim for that carrier stating: additional documentation upon request and send in the report to satisfy their requirement.
