Telehealth Clinician billing site location

In the attached NC Medicaid 1 H telehealth policy it states that the distant and originating site have no restrictions. I am trying to determine if clinicians have to be credentialed at both the distant and origination site when the client is at a SA walk in clinic and telehealth is being provided by a clinician located at another site within the same company. I also understand the place of service will be 11 but what would the billing location be, the origination site or the site the clinicians is credentialed?
