Wiki Telehealth Evaluation & management- can we bill telehealth service for medicare payor? If yes please share the cpt with modifier

In the encounter sheet there is only information of telehealth duration, with the diagnosis. There is no information of originating site. How can we bill this telehealth for medicare payor.

You haven't given enough information here to be able to answer or recommend any codes or modifiers. If you are just working from and encounter sheet with no additional information, I think you will need to talk with the provider and find out more about what kind of telehealth services they are providing, where and how this is occurring, and then research whether or not and how these can be billed.
