during telehealth visits do the Dr's have to document the time spent on there progress notes

It depends on if the code your billing is time defined. Some of the telemedicine codes are time specific in those cases it would be necessary. However, If the you are billing for a regular office visit, such as a 99213, then the documentation would be based on either standard leveling rules or time.
