Does anyone have a link to the documentation requirements for telemedicine visits that include HPI/ROS/Exam or SOAP style notes?
 
Hi there, if you mean the new 980## codes I don't know of any official documentation requirements. Medicare isn't covering them so you'd need to check with each payer to see if they have any guidance. As a general guideline I would follow whatever you're doing for 99202-99205, 99212-99215 visits because the codes are based on them.

Providers should make appropriate modifications based on the type of connection. For example, any exam documentation for an audio/video encounter must make sense. For audio-only services the provider must document medical discussion time.
