what age does a child have to be present (and interact with the provider) for the telephone session
 
Not sure I understand your question exactly. But if you're referring to consent to treatment for children, that is governed by state law, so you'd need to research the regulations of the location where your provider practices to get the answer to this. I'm not aware that anything has been published specifically regarding this situation as relates the COVID-19 situation.
 
I would think this would differ for different specialties, and how the patient is interacting vs how they would in office.

A pediatric visit for an infant with a cough I wouldn't think would require much interaction. A psych visit for an older child might require more interaction
