When billing these codes to MCR such as 99441-99444, which of the "G" modifiers are most appropriate--- GA, GX, GY, or GZ? Would GX be more applicable for when the MCR crosses over to secondary. Would we also use the GX for straight Medicare? What might happen if we don't utilize the modifiers at all?
