Wiki Tenotomy and neurectome help!!!

OP NOTE: Zigzag excision made over the wrist, median nerve identified and neurectomy performed. I identified the pronator palmaris longus and released it. I identified both the FC and FCR tendons and released them. I then went tot he FDS and FDP tendons. I appropriately lengthened them and did a FDS to FDP transfer. I transferred the index FDS to FDP, the middle FDS to FDP, and ring and little FDS to FDP. After I did this, I then did a quick release from the tension sutures and ____ together. At this point, I identified the FPL, using proper techiniques I then lengthened and sutured.

The codes used are: 24640, 26455, 26455, 26498, and 64999.

Would these be the correct codes for this case? Any help is greatly appreciated.
