Patient has Blue Cross insurance. Dr. ordered free and total testosterone, with only diagnosis being fatigue. insurance paid on total testosterone but is denying the free as not medically necessary. is there a frequency restriction on regular insurance? I have looked at our local blue cross medical policy and it did not say anything specific with these labs.
