Please help! what would the appropriate way to bill this scenario be?

PA states: I had a patient that was checked in for puncture wound to the foot - stepped on a nail. Needed Tetanus updated. She was checked in completely with vitals and everything. My staff didn't realize I hadn't seen the patient yet and went ahead and gave her the Tetanus and sent her home. Do I no charge her visit? Would it be considered a nurse only charge?
