Therapist Travel Charges - Home Health

How does a home health agency charge for the travel time of a PT, OT or Speech therapist going to a patient's home to deliver therapy ?

Is the travel time billed seperately or is it figured in to the therapy charge ?

I would appreciate any replies on this question. Thanks :)
