I work in a FQHC (Federally Qualified Health Center), we recently started providing MH/BH services in addition to medical at our facility. One of our providers, he is an LCSW/LAC, saw a couple patients in the local nursing home. I did not realize this was happening until after the fact and I have no idea if we can or how to bill Medicare for these visits.

If we can bill for these services, I need to know what form to send it on, what bill type (if UB), if we can use normal therapy codes (ie 90832), what the point of service would be, and probably a hundred other things I am not thinking of right now.
