I am having a problem with one of my providers, he is constantly billing based on time on ALL his encounters. I do not think that it is okay to always bill based on time; I thought that billing based on time was on those rare occasions when counseling was needed, but I could be wrong. I am wondering if anyone knows any resource I can use that would state this.

He already add the correct documentation to time based coding, he states what he did during his face to face time, but it usually a replica of the previous encounters.

What can I do?
