Do any of you have a description of "Coordination of Care" for time based E/M coding? What does this consist of? I couldn't find a description in the 1995 E/M guidelines. Thanks!

I can't find anything from CMS right now (although I could've sworn I've come across it in the past), but there's a good description of what I would expect to see here:
