Will 99291 and 99358 be supported if the provider documents his time as "at least >30 minutes"

If I was auditing, I might let this go once, but would not accept this as a regular practice. The provider should state how much time they spent, to the best of their ability. Perhaps you could ask the provider how useful they would find it if, for example, a lab sent them a report that said that their patient's blood sugar was at least 95?

Is that time documentation supporting the critical care time or prolonged service?

Time spent "engaged in work directly related to the individual patient's care" is included in critical care per CPT guidelines.
