Good Morning,

I was wondering if there is anywhere in writing that states an appropriate time frame in closing and signing off on documentation?

I have a mental health provider who basically goes thru the notes once a month and signs them. I can tell them til I am blue in the face but they want to see proof that there is a time frame notes should be signed.

I have been seeing more notes where Clinicians are choosing E/M levels based on time, but the note has an electronic signature for 2 days or so after the DOS. This makes me nervous that an outside auditor would question the time vs work on the DOS. Is there an official guideline that I could...
