Wiki time span for diagnosis codes that need to be resolved

I am working in long term care which is all new to me. I was wondering if anyone had pointers concerning diagnosis' that need to be resolved in a timely manner or knew where I might find information concerning resolving codes. I find a bit of information on UTI and pneumonia but there are many others that though it may be acute upon admission may not be acute in 15 to 30 days. Admission dx are not supposed to be changed but what do you do when the admission dx used is a (EX) pneumonia code that has now resolved? Any suggestions or pointers for LTC would be greatly appreciated...
