Is this time statement “a total of xxx minutes was spent today including face to face and non face to face time with patient reviewing relevant history results and records, chart preparing and coordination of care. This excludes time spent on preventative services or procedures.”

If this is just an E&M office visit is the “this excluded time spent” still appropriate to document?

Templates statements such as this are common practice. As long as the statement is accurate and the rest of the note contains detailed information that is specific to the patient and the encounter, I can’t imagine that any auditor would have a problem with this.
