If provider does everything in code 58570 except for using endoscopic tools for removal and remove parts through vagina, how would you code this? Would you use modifier? If so which one? everything else is done the same. Just remove parts through vagina and not with endoscopic tools.
 
I always code as TLH 58570 whether they remove the parts through the scope or the vagina. Most likely the uterus was to large to fit in the pouch to be removed. Make sure you check the gram weight of the uterus and if BSO done last digit will change. Hope that helps.
