Reinsertion of segmental instrumentation, L4 to sacrum. Implants are Nexus TLIF implant sniper 7.5 x 35 length screw left S1. The rest were 7.5 x 40 length screws L4 and 45 length screws at L5, 45 mm precontoured rod on the right, 80 mm precontoured rod on the left, set screws x5.6. Interbody fusion cage L4-5. Implants Nexus TLIF implant 11 mm short rotatable interbody fusion cage x1.7. Local autograft L4-5.8. Operative fluoroscopy.
