Hi, can someone help me remember if when there is a "/" between two diagnoses in the dictation, i.e. emphysema/copd, should both diagnoses be coded or neither. I seem to recall the "/" means "either/or" so neither would be coded.

Whenever you are uncertain as to the meaning of any documentation - it is always best to return to the provider for clarification.

I would find the "/" to mean, like you, "either/or" and would seek clarification from the provider.
