Question?last week I scheduled a TOC appointment for 11/26 in anticipation of one of our patients being discharged from the SNF on 11/15. Will it still count as a TOC even though I did not document contact within 2 days of discharge since we already knew when she was going to be discharged and had scheduled the TOC?
