If is noted in Surgical History "Toe amputated, (L)" and patient is there for Follow up on Chest pain. For Commercial Risk Adjustment Amputation generates an HCC and must be documented yearly to obtain HCC so if Physician noted ?Amputation site looks good or normal" . Can you code the toe amputation? Other than that, I do not think you can code with it just in Surgical History like that? Does anyone disagree? Thanks
 
Yes, as long as it is somewhere in the documentation amputations and ostomy status does not need to be addressed like other conditions. Usually we adhere to the MEAT philosophy, but these are the exceptions to that. As well as V58.67-long term use of insulin.
