Hi, Anyone can clarify the below scenario.

I understand that 88333 will be coded for the initial site Touch prep and 88334 for additional site.


Please advise how will you consider and code the below case.

Shall I code them with 88333 - 1
or 88333-1 and 88334 - 4

Do we consider here each block as additional site?

Intraoperative Consultation
TPA 1-5 "SP1 left Sentinel lymph node" adipose (3.7 x 3.2 x 1.5 cm)
5 lymph nodes:
TPA - 0.8 cm
TPA2 - 0.9 cm
TPA3 - 1.2 cm
TPA4 - 1.0 cm
TPA5 - 0.8 cm

Touch preps prepared.

Gross Description
Container A: Following intraoperative consultation, the specimen is received in a formalin-filled container labeled with the patient's name (visually verified by ESS) and additionally labeled "SP1, LT sentinel lymph node" and consists of 5 lymph nodes submitted in 5 white cassettes labeled "TPA1" - "TPA5" (0.8-1.2 cm in greatest dimension.) Additionally within the container are 2 pieces of yellow-tan fibroadipose tissue (4.2 x 1.5 x 0.4 cm and 2.0 x 1.5 x 0.3 cm.) The specimen is entirely submitted in following cassettes:

55-SP-16-8097 A1: Lymph node, TPA
55-SP-16-8097 A2: Lymph node, TPA2
55-SP-16-8097 A3: Lymph node, TPA3
55-SP-16-8097 A4: Lymph node, TPA4
55-SP-16-8097 A5: Lymph node, TPA5

I think this can be 88333 X 1, 88334 X 4. Each touch prep was done on a different lymph node, so that certainly qualifies for "additional site."
