How would you code the scenario below?

The tube actually exits the trachea and the opening starts to narrow causing potential airway obstruction and the physician has to dilate the stoma and re position a trach tube, which is not simply a regular trach change but rather dilation of the stoma and re-positioning a tube into a closing trach stoma in an emergent setting.

Your assistance would be appreciated.

It is hard to code this without the operative report as it depends on the instruments used:

Was the procedure done using a laryngoscope?
A bronchoscope?
If a bronchoscope, was the scope advanced into the bronchi?

Was the original tube placed by your surgeon?
If yes, then was the dilation/repositioning performed outside of any global period?
