Wiki Transfer in at 23 weeks? Split global or E/M?

Hi everyone!
I've read up on splitting the global codes 59425 4 or more, 59426 7 or more, followed by just delivery code, but I guess I need help. Quite often our practice will have a pt tansfer her ob care to us at 23, or 25 weeks. I understand at the end it will be 59426, +del code, BUT For her initial visit to us, can't we bill an new pt E/M (99204) for this visit to establish care? But what to use for dx code? Can anyone give me an answer on this, My ACOG resource isn't helping! GRATEFUL THANKS!
 
That visit would fall under the OB (59426). It would be like a patient coming to your office for their 1st OB visit it is part of the global package.
