Does anyone have any insight or resources on the E&M points for History of Present Illness (HPI) for transgender visits?

We have identified the following points:
Context = initial or follow up visit
Onset = Age of awareness
Duration = Transitioning for how long
Associated signs & symptoms = presenting symptoms
Modifying factors = aggravating or relieving factors

Any thoughts on Location, Quality, and Severity?
