I'm confused. I keep seeing where people say their claims are being denied for "time", and that these codes shouldn't be billed for 29days after discharge? I thought they were supposed to be used 99495 within in 14 days of discharge and 99496 within 7 calendar days?. So are you suppose to use a regular E/M codes to see them first and then after 29 days has gone by you can use these codes? Please help. Also it looks like you are to of had some type of "direct" contact with them within 2 days of discharge. Soooo confused. Thank you
