I have billed 99496 transitional care with modifier 25 and 99406 smoking cessation.
New 2016 guidelines have stated that transitional care can now be billed on the day of the face to face visit that's why they were billed together on the same day. I received a denial saying the procedure code is inconsistent with the modifier used or a required modifier is missing. Any help as to why or how this should be billed would be greatly appreciated.
