I am billing for TCM using CPT 99495. Documentation has met requirements. Dates of Service on claim shows discharge date 10/18/13 as day 1 (start date) and 11/16/13 as day 30 (end date). NGS Medicare in Illinois. Claim rejected stating it needs two lines of entry because it involved two different months for the 30 days of coverage. How do I bill this? Has anyone been successful in billing TCM services to NGS Medicare in Illinois? Thanks!
