I work for a hospitalist group and I am trying to figure out if Transitional Care Management is going to apply to us. We do not have a clinical setting to follow the patients after they have been discharged from the hospital. Any help would be appreciated!!
 
No, TCM would not apply to you. TCM is designed to allow primary care offices the ability to bill out for Nurse Care Manager services to prevent re-admission of their chronically-ill patients. As a hosptialist group without a primary care panel, and as the providers who care for the patient in the inpatient setting, your only role will be to provide clear discharge summaries and discharge planning so that the care managers can work with the patients.
