Our coding/compliance teams have been going around on this and I'm hoping for some input.
To bill 99495 or 99496, we've understood it to be 2 business day contact, 7 or 14 day visit, and level of MDM.

The question is - do these codes also require documented consent from the patient? I haven't found anything yet that says they do, but compliance is telling us that we have to have consent and be able to provide the cost of TCM vs office visit.

Thoughts?
 
There is no separate consent needed for TCM visits that I'm aware of. I'm attaching the 3 best references I've found for TCM. Hope they help. :)
