I would like to know what you bill when the patient is seen within 7 days of discharge but the the MDM only meets the level of moderate complexity and not high. Just bill E/M's and forget TCM?
 
Was the patient returning during a global period for the same reason? If not then I think what ever the documentation states still has full weight.
 
If all other TCM requirements are met during 30 day post-discharge period (in addition to the face-to-face within 7 days), you would bill TCM CPT 99495. MDM must be high in order to bill 99496.
