Wiki Triamcinolone denials

We have begun receiving anthem denials for J3301 for Triamcinolone when billed with NDC 16714015001. This is the NDC on the bottle and the NDC crosswalk shows that we should bill it with J3301 and yet we are getting tons of denials stating that the code and NDC combination is not correct.

Is anyone else dealing with this? Am I missing something that happened? This has only started in the last two months. Please help!
