Wiki Triple Osteotomy on right hip

How do I code a triple osteotomy on the right hip? The op report specifically states three separate osteotomies were performed during the same operative session all on the right hip. Even when I append modifier 59 for separate incisions, I still get the same error that it is anatomically impossible to perform 27146 more than twice in one day because we humans only have 2 hips.

Did he actually do them all on the femur? Some surgeons say hip when actually acetabular osteotomies done.
