Is anyone having difficulty getting multiple levels of an RFTC to pay by Tricare? We did RFTC of the L Facets L3-4, L4-5, and L5-S1 bilaterally and we cannot get Tricare to pay the second additional line. This is a 2011 claim so we are billing 64622-50, 64623-50, and 64623-50-59. Please help!
