When coding a TURBT Op Report, for proper documentation, does the tumor size need to be reported in the Procedure Description AND in the Details of the Procedure? Most of the time the surgeons do not list the tumor size when describing the surgery in detail, they only list it in the Procedure Description.

Hello, for all the ones I have coded, my reports show the actual documented tumor size the MD is taking out for that particular patient, they're not noted by CPT descriptor range.
