I have not run into this one before. This is a 34 wk PROM twins vaginal delivery. Two docs involved, one did the actual deliveries and the note states "...followng the delivery of twin B with the assistance of Dr. C on ultrasound to make sure twin B was in the vertex position, the placenta was delivered spontaneous..." (not a typo on my part here)
Wouldn't this just be an ultrasound w/mod -26?
 
Yes , exaclty, that is what I would opt for; the doctor C will be eligible for US Code with mod 26
