Physician performed a nursing home admission. On the same date, the patients condition deteriorated and the nurse practitioner from the same group was called in to see the patient. Are we able to charge for the nurse practitioner visit? Same primary diagnosis, but additional manifestations. If so, any modifier?
 
I would combine the documentation from both visits, and calculate the admission based on the elements of both documents. You can try to bill out two visits and modify one E&M, but typically Medicare pays only one E&M visit per practice specialty per day within a facility or office setting, so you'll likely get a denial if you do, particularly if it's for the same primary diag. Better off to bill only one service.
