1. Dr. sees pt in the hospital for a consult; then dr sees pt in the office (after pt d/c) - we would bill an established pt visit for the OV, correct?

2. Dr. sees pt at one location. Then Dr. leaves that practice and joins a new practice. The pt then comes to see the dr at the new practice (within three years) - we bill established pt visit for the OV, correct?

I have been trying to find some documentation to support these but I wanted to get others input as well.
