Wiki UB Box 70 claim denial

Hello,
We bill out on the UB for our ASC claims. We are a free standing ASC. We are having our claims not processed with Anthem due to reason code
207 Outpatient Patient Reason DX Code is blank invalid or illegible. They want box 70 filled in, we have never done this before, and as far as we know this is only a requirement for the hospital. Any one else dealing with this?

Yes our ASC is rec'ing this denial for Medicaid Anthem and trying to explain this is for "inpatient"...Still trying to work this out.

We have called BC about this several times, and researched it on the website. Apparently they had an update in March, and they are now requiring that box to have the pre-op dx, or admitting dx on facility claims .

Hello everyone, just a quick note on the above issue. We are also an ASC and having troubles with BC. I would like to know if you have yet to receive the next line of denial that would be box 74 Invalid Principle Procedure Code? Also is there a hardcopy update from March any help would be appreciated. Thanks
