Wiki UHC CLAIMS DENIAL FOR CPT 90460 & 90461

I've been getting claim denials from United Healthcare for CPT 90460 as an add-on code, which isn't an add-on code.
I've called UHC and asked to reprocess these claims and I keep on getting this rejection.
Is anyone else having this problem?
