For UHC Medicaid we are having issues obtaining payment for Physical Therapy. We have tried billing the codes the just the GP modifiers and with both the GP and AT modifiers. Does anyone have any other ideas? Or has anyone had any success when billing UHC Medicaid for physical therapy?
