Please can anyone help? I cannot find an LCD for this procedure code, but I have access to CustomCoder and the printout from that gives me a certain set of codes that are considered medically necessary.

Is there an LCD or is this one that is done on a case by case basis and DX?
Thanks for any help you can give.
