Is anyone requiring a HARD COPY to be documented in the op report when US guidance is used, say for a breast biopsy? We received a notification from a radiology newsletter that one of the requirements to report 76942 is a hard copy of the ultrasound (imaging supervision and interpretation per CPT book). There is always a description of the procedure, but not always notation of a storable copy on the chart. Please advise.

Thank you for your reply. Did an audit company make that request or is it a facility procedure, etc? Just curious.
