Wiki Ultrasound guidance, 76942

Is anyone requiring a HARD COPY to be documented in the op report when US guidance is used, say for a breast biopsy? We received a notification from a radiology newsletter that one of the requirements to report 76942 is a hard copy of the ultrasound (imaging supervision and interpretation per CPT book). There is always a description of the procedure, but not always notation of a storable copy on the chart. Please advise.

We are required to keep a hard copy of all 76942's on file and accessible to be sent to insurance if requested.

Thank you for your reply. Did an audit company make that request or is it a facility procedure, etc? Just curious.
